On 21st of March 1991, Loaiza signed to Pittsburgh Pirates as an undrafted free agent. In the Rookie leagues, he started 5 games, finishing with a 5-1 record and a 2.26 ERA.
The following year, he switched to class A, Augusta. Further, he played for Salem and Carolina. Also, he represented the Mexico City Red Devils of the Mexican Summer League during the 1993 season.
In 1995, he moved back to Pirates, making his major league debut against the Philadelphia Phillies on 29th April 1995. The first major league win of his career came when he worked 4 2/3 innings in the game, giving up 5 hits and 1 unearned run.

On 17th July 1998, Loaiza was traded to Texas Rangers in exchange for Warren Morris and Todd Van Poppel. He played the two seasons with Texas, pitching both as a starter and a reliever for the Rangers.
On 19th July 2000, Loaiza was traded to the Toronto Blue Jays for Darwin Cubillan and Michael Young. Through 2002, he pitched with Blue Jays.
In 2003, Loaiza signed for the Chicago White Sox as a free agent. In his first season with Sox, he led American League pitcher in 207 strikeouts as well as ranked second behind Roy Halladay, ahead of Tim Hudson and Pedro Martinez. In 2004, Loaiza was voted to the All-Star games as a member of the White Sox.
In mid-season of 2004, Loaiza was traded to the New York Yankees for Jose Contreras.

In 2005, Loaiza signed for the Washington Nationals agreeing on a 1-year deal as a free agent. In that season, he went 12-10 with a 3.77 ERA for the Nationals.

On 14th June 2006, Loaiza was arrested after being pulled over by police who clocked his Ferrari at 233km/h on a Hume freeway near San Lorenzo. Also, he failed a sobriety test. Following the incident, he was banned to consume alcohol in both the home as well as visitor clubhouse.
